Discharge Service
The Grace Consulting Patient Discharge Service accelerates the speed
at which hospital patients needing ongoing care are discharged into care
homes or care at home.
This service works alongside your existing arrangements and significantly
reduces the incidence of delayed discharges, thereby saving Social Services
departments money under the provisions of the Community Care (Delayed
Discharges) Bill and providing an efficient service for its clients.
