A free service to hospitals accelerating the speed of your hospital discharges

The Careseekers Patient Discharge Service is designed to accelerate the speed at which self-funding elderly patients needing ongoing residential care are discharged from hospitals. Careseekers is a separate stand-alone division of Grace Consulting.

What does the Careseekers service provide?

CareSeekers provides a free service to patients and hospitals, finding appropriate vacancies in care homes for privately funded patients needing discharge into care.

We offer a concentrated service, working in tandem with the hospital discharge team and the families of the patients, quickly finding patients and their families appropriate vacancies in care homes and helping to ensure a quick and efficient discharge from hospital.

The service helps hospitals to reduce delayed discharges and is completely free.

We are entirely independent and always work in the best interests of the patient. As an example of our speed, assuming we can talk to the people concerned, we typically advise the families about ideal vacancies on the same day or the following day of the hospital referral to us.
